You’re welcome to stop by during our walk-in hours at either our Columbia or Lexington location for an assessment with one of our clinicians.

Adult Services
What to Expect
During intake, the patient will receive general information about the adult services division and will be presented the basic paperwork needed to begin the assessment process. A counselor will then meet with the patient for an assessment and drug screen. Based on the assessment, the Treatment Team will determine the appropriate recommendation.
What to Bring
- Identification Card (ex: picture identification)
- Insurance or Medicaid card
- Official printout of current medications
- Any referral information

ADSAP

What to Expect
Administrative staff will complete an intake which includes forms that address policies, procedures, and financial responsibilities. An ADSAP clinical counselor will conduct a screening and/or assessment to determine the focus of services and the level of care that will best meet your needs. Clinical staff will then orient you with the Alcohol and Drug Safety Action Program (ADSAP) requirements.
What to Bring
- Identification Card (ex: picture identification)
- Insurance or Medicaid card
- Official printout of current medications
- Any referral information
- 10 year driver’s record dated within 30 days of enrollment
- A $500 payment is due at the time of your assessment. If you’re unable to pay at that time, you’ll have the option to apply for Community Service or set up a payment plan.
Youth & Teens
What to Expect
During intake, the parent/legal guardian and the child will receive general information about the adolescent services division and will be presented the basic paperwork needed to begin the assessment process. A counselor will then meet with the patient for an assessment and drug screen. Based on the assessment, the Treatment Team will determine the appropriate recommendation.
What to Bring
- Identification Card (ex: picture identification)
- Insurance or Medicaid card
- Any referral information
- Copy of your child’s immunization record
