Where is LRADAC located?
LRADAC has two campuses.
The Richland County campus is located at 2711 Colonial Drive in downtown Columbia.
The Lexington County campus is located at 1068 South Lake Drive in Lexington.
My Assessment: When to come? What to bring? What to expect?
LRADAC provides assessments to determine the appropriate level of care for Adults, Youth & Teens and ADSAP participants. Walk-in assessments are available Monday through Friday from 8:30 a.m. to 3:00 p.m. at both our Columbia and Lexington locations.

Does LRADAC protect my privacy?
Your health information is personal. As an accredited treatment provider, we comply with state and federal regulations which require us to maintain the highest standards of confidentiality. Here is a statement of our client rights.
Staff, patients, family members and visitors all sign a confidentiality statement and agree to keep all knowledge of information of LRADAC’s staff, clients and family members confidential at all times. Confidentiality may be breached if LRADAC suspects past or present abuse or neglect of children, adults and elders or if there is potential danger towards harming oneself of another individual.
How do I access my records?
LRADAC maintains patient records. Patients may submit a request to review, copy, or make amendments to their treatment records or to have a copy of their records sent to other specified individuals or organizations.
- Patients must complete the General Release of Information Form prior to the request being completed when the information is to be released to anyone other than the client.
- All requestors must provide 72-hour notice to allow time for the request to be approved and records to be collected.
- Email requests will not be accepted.

Per policy, LRADAC will provide patients and other authorized requestors with copies of their records upon written request and in accordance with all applicable state and federal regulations. Patients may be charged for copies in accordance with the agency’s financial agreement. Other entities requesting copies of patient records with the exception of state and federal agencies, rehabilitation facilities, and other health/social service providers will also be charged in accordance with all related regulations and/or contractual agreements. If payment is required, that payment must be made prior to the release of any information.
Only the minimum necessary information will be released in response to each request. Requests indicating “any and all,” “all records,” “all dates,” etc. must be clarified with the requestor to determine what information is being sought.
LRADAC is prohibited by 42 C.F.R. (Code of Federal Regulations) Part 2, Final Rule from responding to a records request or from confirming or denying that an individual is receiving or has received services from LRADAC without a properly executed release of information.
Insurance & Payment FAQ
LRADAC provides services regardless of my ability to pay. Does this mean LRADAC is a free clinic?
No, LRADAC is a not-for-profit organization. While LRADAC does receive some state and federal funds from grants and contracts, those funds don’t cover all of LRADAC’s operating expenses. Therefore, LRADAC does charge and bill for its services.
How much do services typically cost?
All fees are accrued based on the most recent financial agreement and fee schedule.
Accepted forms of payment included self-pay, insurance coverage in accordance with the patient’s policy, Medicaid, and financial aid in the form of the LRADAC Recovery Assistance Fund.
What insurance does LRADAC accept?
At LRADAC, we serve any individual or family in Richland or Lexington Counties regardless of financial resources or insurance status. Once you or your loved one have decided to seek treatment, our assessment team will make a recommendation concerning the correct level of care required. Final cost of treatment is determined by level of care required and length of participation.

We ask that you call your insurance company by using the customer service number on the back of your insurance card for specific coverage information. For other questions about insurance, payment options or financial assistance, please call (803) 726-9393 and ask to speak with Patient Accounts.
What if I don’t have insurance?
LRADAC provides services regardless of an individual or family’s ability to pay. You can walk in during our assessment hours, Monday through Friday 8:30 a.m. to 3:00 p.m., to receive an assessment for LRADAC services. During this assessment, you will meet with our Patient Accounts staff who will learn more about your financial situation and help you apply for assistance.

What kind of financial aid is available?
Thankfully, because of charitable contributions to the LRADAC Foundation, individuals and families without insurance or those facing financial hardships can apply for financial assistance to help pay for assessments, treatment and other programs. During your assessment, our Patient Accounts staff will provide you with information about recovery assistance, eligibility for financial aid, and how to apply.
Do you have a payment plan option?
As a not-for-profit treatment provider, LRADAC believes everyone should have access to treatment services. If fees exceed your ability to pay, we will work with you individually to develop a payment plan so that financial restraints are not a barrier to your recovery.
Youth & Teens FAQ
How long is the course of the program?
The length of the program is based on the individual’s needs. Once an assessment is completed, the counselor will meet with the patient and their family to talk about the recommended course of services. A patient’s engagement and their progress on goals will also determine the length of time they are in services.
Will attending services conflict with the school day?
Group times are offered in the evenings after school. Assessment times may be scheduled and walk-in times are available.
Will a parent/guardian need to accompany the patient to each appointment?
For individuals aged 16 and older, parental participation is not required—but your support is always welcome. While we encourage family involvement, our priority is making sure teens feel comfortable seeking help, even if they choose to do so independently.
